Parent Input Form: Student placement is a team decision based on student academic performance and needs, behavior, teacher style, service needs, and classroom designs. At CES, we welcome parent input regarding their child’s learning environment. We do not accept requests for specific teachers. We want you to know that your request will be given serious consideration, however completion of this form does not guarantee a specific class placement.

In efforts to address frequently asked questions for families of incoming kindergarteners, we offer an informative brochure along with a Kindergarten Orientation presentation to provide answers and insight to our school. We invite families looking to enroll their first and second graders to review the information as well.

Enrollment Health Requirements:
All students entering CES will need to submit a valid physical examination completed by their physician to enter school. Pre-K students need to have had a physical completed within the 12 months prior to the first day of school and then submit an up-to-date physical every 12 months. Kindergarten students need to have had a physical completed within the last 12 months prior to the first day of school. Grades 1 & 2 students need to have had a physical completed within the 12 months prior to when they entered kindergarten. Please have their physician complete the most current State Health Assessment Record for your child's grade. For your convenience, copies are provided in our enrollment packets, as well as on our website (see links below). Please note that Pre-K students DO NOT use the same form as K-2. Pre-K uses the "yellow form" and K-2 uses the "blue form".
For Pre-K, the State of CT Early Childhood Health Assessment Record (see "forms" below) must have the first page completed by a parent/guardian and the following two pages completed by their physician.
