Registration & Residency
Welcome to Colchester and the William J. Johnston Middle School. We look forward to the opportunity to work with your child. For your convenience, below you will find the necessary paperwork required to enroll your student. By clicking on the links provided you will be able to print and fill out paperwork at home, however, enrollments must take place in person at our school office. Please call the WJJMS main office at 860.537.2313 to schedule an appointment with the appropriate School Counselor and complete the enrollment process. You will also find below the information on how to make changes to or update your student information form and see the directions on how to withdraw your student as well.
The Colchester Board of Education requires that each new student registers and provides proof of residency prior to attending Colchester Public Schools. Current Colchester students who move within town must also provide proof of residency. To register within the Colchester Public Schools, we require two Colchester, CT proofs of residency.

Your child's most recent in-state medical, physical and immunizations record (subject to approval by our School Nurse)

You will need to fill out an enrollment packet from our office to complete all of the necessary forms and furnish additional documents required for enrollment at WJJMS. We can, however, assist you in obtaining your child's academic and medical records as you provide us with your child's previous school's contact information. The additional documents that we require are as follows.
-your child's birth certificate with a raised seal
-most recent copy of your student's 504/IEP (if applicable)
-most recent copy of your student's report card and current quarter or term grades
-your student's previous school's contact information

As you enroll, current cafeteria news, meal assistance application forms, school maps and pick up/drop off procedures will be handed to you and explained.
Make changes or update your student information form
Click here for the student information form if you need to make changes in your contact information to keep our office informed. You may send the form in with your student, stop by our office, or mail it to 360 Norwich Avenue, Colchester, CT. 06415.
Withdraw your student
If your student is leaving our Colchester School District, click here for the authorization to release student records from William J. Johnston Middle School form. Please return the form to our school office at your earliest convenience. We would be happy to assist you and forward the necessary paperwork to assist in the enrollment process at your student's future school.
