Colchester Public Schools

The Connecticut State Department of Education mandates that screenings be conducted by your child’s school nurse in the following grades:

• Hearing – grades K, 1, 3, 4, and 5
• Vision – grades K, 1, 3, 4, and 5
• Postural (scoliosis) – female students grades 5 and 7. Male students grade 8

If a referral is necessary the school nurse will inform the parent/guardian in writing. It is the responsibility of the parent or guardian to schedule the appropriate referral and to provide follow-up information to the school nurse. If assistance is needed to accomplish this, parents/guardians are encouraged to contact the school nurse.

If you do not want your child screened for vision, hearing, or scoliosis, please your child's school nurse.
